Our Wish List to Help Us Work As a Team

Please arrive on time for your appointments and if you can not make your appointment, please call within 24 hours to cancel. Notice less than 24 hours notice will be charged a missed appointment fee of $25.00. We have an automated system call you to remind you of your appointment.
Late Patients: Patients are required to be on time to their appointment. If possible patients should arrive a few minutes early to check in and update their information. If a patient is more than 15 minutes late for an appointment, the appointment may be cancelled. It will be at the discretion of the provider and the front desk staff to determine if there will be enough time to see the patient without making other patients wait. A cancellation fee may be charged if your appointment has to be cancelled
Please understand that we have all patients return for skin biopsy results. This is our policy and results will not be given over the phone.
We utilize a dermatology billing agency to process your claims and bills in an orderly up to date fashion. If you have questions you may call our billing agency at 1-888-245-5337 (Toll Free). If your issue is not resolved, you may call our office manager.
It is of the utmost importance that we respect your time and run our office in a timely fashion. In the event of an emergency add on or complicated case, the front staff will make an announcement to the patients in regards to the delay. We promise that we will give you the same careful attention.
Follow-Up appointments are scheduled with our Mid-Levels unless specifically requested by you or the doctor. Our physicians are always available for any questions.
Prescription Refills and Nursing questions (Prompt # 3) will be returned between the hours of 12:00pm—1:00pm and again between 4:00pm—5:00pm. True medical emergencies, please press #1 on our phone prompt to be directly connected with a staff member. We have a 24 hour on call service for “True Medical Emergencies”. Please call our main number (410-472-1006) and leave a message. The provider will call you back within the hour. Please note, this is only for “True Medical Emergencies”.

Necessary Forms

Our patient intake forms are available for download. Save yourself some time and complete prior to arriving at your appointment. Bring all your forms with you along with a copy of your driver’s license and insurance card. If you are unable to make a copy of your insurance card and driver’s license, we can do it at the office. All forms are required.

Referral Requirements

If you are insured under an HMO, you are required to obtain your referral prior to your visit. If you did not request a referral, you may either reschedule your appointment in order to obtain the referral, or you may be seen as a self pay patient accepting services delivered outside your insurance coverage. If obtaining services outside your insurance, a waiver must be signed before the services are rendered.

Patient Referral Waiver

If you do not have an insurance that is listed above, you will be required to pay for your visit in full. We will provide you with the appropriate statement to submit to your insurance company.

Billing Questions?

DBA Billing Service, Inc. out of Casselberry, Florida has been retained by our office to handle all billing and processing of claims to insurance companies. Please make all your payments to North Baltimore Dermatology if you were seen in the Sparks office or Advanced Dermatology, Bel Air if you were seen in the Bel Air office. If you have questions about the status of your account, please call 1-888-245-5337. They will have access to all billing information.

Laboratory Billing

We send tissue to an outside reference lab of our choice for slide processing and interpretation. Patients and their insurance companies may receive a bill from the outside laboratory. Any billings you receive should be handled with the insurance company and the laboratory directly.
